2. Print out then fill out the pdf form Participant consent and waiver form and email it to Marg at eagleandcondorcollective@gmail.com or bring it with you. We will not allow any participants without the form due to insurance reasons. Also if you don't have a printer we can send one via google docs for you to fill out directly online. Thanks!
